Ho’okele ABA may use your mobile phone number to communicate about scheduling, pertinent patient information, or upcoming events. We obtain consent for messaging in several ways and have several options for removing yourself from our messaging service: 

A mobile user might opt-in by:

Entering a phone number online,

Sending an Mobile Originating (MO) message to our main line or extensions,

Filling out a paper form that includes their phone number, or

Signing up at a point-of-sale location.

Opting out

Ho’okele ABA gives you the ability to opt out of the Service for any reason. You can opt out by texting “STOP” or “QUIT” to any message received. You can send an email to scheduling@hookleaba.com requesting removal from messaging. You can talk with a representative at any location. You can call the main line, 808-556-3120 and leave a message 

We reserve the right to send you certain communications relating to the services, such as announcements and administrative messages, without offering you the opportunity to opt-out of receiving them. 

Data

We may collect personally identifiable information that you provide to us, such as your name, address, phone number or email address. With your permission, we may also access other personal information.

We do not knowingly contact or collect personal information from children under 13. If you believe we have inadvertently collected such information, please contact us so we can promptly obtain parental consent or remove the information.

Access

You can request to see or delete your personal data. You can see or delete any personally identifiable information we have stored, such as your name, address, email or phone number. You can also contact us by email to request to see or delete this information.
